The Fortitude & Freedom Process:

Step 2
Step 3
Book a FREE Discovery Call or Movement Screen
Click on any "Book FREE Discovery Session" button to set up a time to speak with a Doctor of Physical Therapy.
Choose "Free Discovery Call" to have your provider call you at that time to discuss your situation in detail in order to provide you clarity about your problem and determine how we can best serve you.
If you prefer to meet in person, book a "Free Movement Screen" where you can discuss your problem in more detail and receive a complimentary movement analysis!
Schedule a 1-Hour Initial Evaluation
Your appointment will always be a full hour
1-on-1 with a Doctor of Physical Therapy.
At the initial evaluation, you will gain a clear understanding of your current problem, the root cause, and the solution to resolving it for good.
We will establish a clear plan of care to start relieving pain, correcting the underlying root cause of the pain, and providing you the skills and tools necessary to keep your pain gone for good.
Carry Out a Plan to Crush Your Goals
Most people complete their plan of care in 5-10 visits spread out over a 3-4 month period.
Your plan of care will include 3 Phases:
1-Relieve the pain. We take the first couple sessions to calm down the injured tissue.
2-Correct the root cause of the pain. When you're feeling better, we start correcting the deficits that caused the pain in the first place.
3-Develop Resiliency. We then ramp up activity to safely return to, or exceed your prior fitness level or activity goals.

Wade S.
"I have a hereditary condition that causes lower back pain, hip pain, and limited mobility. I saw Jake to help with an incident that caused me to throw my back out. I was experiencing pain, discomfort, and very limited mobility. He was very personable and wanted to understand not only my current situation, but also my underlying condition and other causes. I felt heard and understood and we were able to discuss my short-term and long-term goals for my back and hip. He was able to assist in identifying the problem areas and worked to stretch and strengthen those areas. I felt significantly better after the first visit and am stronger and more flexible now due to the exercises and recommendations he gave.
I found Jake’s personality, professionalism, and ability to identify my issues to be exactly what I was looking for. I view him as a trusted therapist for any needs I have in the future. I would wholeheartedly recommend him to anyone in need of physical therapy services."
